All providers listed with Freedom Providers (East Riding) have agreed to follow our Quality Standards and are subject to our Quality Assurance Process.

Complaints

If you have a complaint about the service you’ve received, we encourage you to raise it directly with the provider in the first instance. All Freedom Providers should have their own Complaints Procedure, which they are required to share with clients at the start of the service and make available upon request at any time.

If your concern relates to the overall quality or safety of the provider’s service, or if you believe they have not met the FP Quality standards, you can report this to us by completing the Freedom Providers Service Quality Feedback Form https://forms.office.com/e/xntyZEzgtS
